A clinic in Dubai is licensed twice: once as a company and once as a healthcare facility. This guide walks the full regulatory sequence in the order the Dubai Health Authority actually runs it, from company formation and the Sheryan application through layout approval, fit-out inspections, the medical director, staff licensing, and the final activation of the facility licence.
Who licenses a clinic in Dubai?
On the Dubai mainland, outpatient clinics are licensed by the Dubai Health Authority (DHA), and every stage of the facility application runs through Sheryan, the DHA electronic licensing platform. The DHA licence sits on top of a commercial trade licence issued by the Dubai Department of Economy and Tourism, so the company and the facility are approved on two separate tracks that have to stay synchronised. Clinics inside Dubai Healthcare City follow the free zone's own regulator instead, and facilities in the northern emirates or Abu Dhabi fall under MOHAP or the Department of Health respectively. The current standards and circulars are published by the authority at dha.gov.ae.
What a licensed Dubai clinic holds before its first patient
- A trade licence carrying the correct healthcare activity
- A DHA facility licence, applied for and tracked through Sheryan
- An approved layout, built out exactly as drawn and passed at inspection
- A DHA-licensed medical director accountable for clinical governance
- DHA professional licences for every physician, nurse, and allied health clinician
- Malpractice insurance, clinical policies, and medication management controls
Step 1: Form the company and trade licence
The legal entity comes first, because the DHA application is filed in the name of a licensed company. The trade name is reserved, the memorandum is executed, and the trade licence is issued with the healthcare activity that matches the intended clinical service. The activity wording matters: a licence issued for general trading or consultancy cannot carry a clinic, and correcting the activity later restarts part of the process. Healthcare activities on the mainland are generally open to full foreign ownership, a point we cover in detail in our guide to foreign ownership of UAE healthcare businesses.
Step 2: Open the DHA facility application on Sheryan
With the entity in place, the facility application is created on Sheryan. The application names the facility category, the intended specialities, and the location, and it is supported by the corporate documents, the ownership structure, and the proposed medical director. The DHA reviews the submission and issues an initial approval, which is the authority's permission to proceed to premises and staffing rather than a licence to operate. Choosing the right facility category at this stage determines everything that follows, and the trade-offs between the categories are the subject of our companion guide, medical centre vs clinic in Dubai.
Step 3: Layout approval and fit-out inspections
The premises track is where most self-managed clinic projects lose their timeline. The unit is leased, and layout drawings are prepared against the DHA's physical requirements for the declared specialities: room sizes and functions, patient flow, accessibility, ventilation, infection control provisions, and the engineering services behind them. The drawings are submitted through Sheryan and must be approved before fit-out begins. Building first and drawing later is the single most expensive mistake in clinic setup, because every deviation found at inspection is demolished and reworked at the operator's cost.
Once the approved layout is built, the facility moves to inspection. A disciplined build treats the official visit as a formality by running a full mock inspection first: rooms checked against the drawings, equipment in place and calibrated, signage compliant, sharps and medication storage correct, and records ready for review.
Step 4: Appoint the medical director
Every DHA-licensed facility must have a DHA-licensed medical director who carries clinical accountability for the facility: its staff, its policies, its prescribing, and its compliance with DHA standards. The medical director is named in the application, credentialed like any other clinician, and verified before the licence is activated. For a new operator this appointment is frequently the hardest single hire, because the candidate must hold the right qualification tier, pass credentialing, and accept the governance responsibility that attaches to the role. In a turnkey build the medical director is placed and licensed as part of the engagement rather than recruited cold by the founder.
Step 5: License the clinical staff
Each physician, nurse, and allied health professional must hold an active DHA professional licence linked to the facility before treating a patient. The credentialing sequence runs through primary source verification of qualifications and experience, any applicable assessment, and the issue of the professional licence once the facility can sponsor it. Staff licensing runs in parallel with the premises track in a well-managed build; run in sequence, it routinely adds weeks. Clinicians already licensed elsewhere in the UAE can often transfer, but the transfer itself is a process with its own documentation, not an automatic recognition.
Step 6: Inspection, activation, and what follows
When the premises pass inspection, the staff licences are linked, the malpractice insurance is in force, and the clinical policies are in place, the DHA activates the facility licence and the clinic may open. Activation is a beginning rather than an end: the licence renews annually, inspections continue, staff changes must be reflected on Sheryan, and services outside the approved scope require an amendment before they are offered. Operators adding pharmacy, medication stock, or regulated products also pick up the supply chain obligations we describe across our registration and importation guides.
How long does it take, and the turnkey route
There is no single official timeline, because the total is the sum of authority processing at each gate plus everything the operator controls: how quickly the unit is found, how many drawing revisions the layout needs, how fast the medical director and clinicians clear credentialing, and whether the workstreams run in parallel or in sequence. Self-managed projects most often stall between layout approval and fit-out, and between initial approval and staff licensing.
The alternative is to hand the whole sequence to one accountable partner. Turnkey clinic builds, with the entity, the DHA licensing, the premises, the systems, and a zero-payroll clinical team delivered under one engagement, are delivered by HealthcareSetup, a Pharmareg UAE company, with launches typically targeted in approximately 45 days, subject to authority processing times. Operators planning an aesthetic, longevity, or preventive health concept can see how the model applies to that segment in the HealthcareSetup wellness and longevity clinic setup service, and founders still weighing structures can start with its explainer on the healthcare business licence in Dubai.
